Required: 17 characters for vehicles from 1981 onward. For older vehicles, enter the identification number. Find it on your registration or at the base of the windshield.

Enter your insurer’s name, “Self-pay,” or “Not sure yet.” No policy or claim number needed.

A short description of your vehicle's repair needs is enough. Please do not include payment details, government ID numbers, policy or claim numbers, medical information, passwords or sensitive documents.
